## Why SRS? Should I consider SRS?
|
||||
|
||||
SRS is not necessary for every transgender person; its direct effect is simply to reshape the reproductive organs into a female form and to gain partial access to functions including penetrative sex, as well as the right to modify the legal gender in some countries.
|
||||
|
||||
The impact of SRS is relatively indirect in terms of social identity, for example. Furthermore, on the basis of good HRT, SRS (or gonadectomy alone) has a relatively limited impact on further feminisation in endocrine terms, and the risks of long-term HRT (even with CPA) are not uncontrollable.
|
||||
|
||||
The [WPATH](https://en.wikipedia.org/wiki/WPATH) guidelines have strict rules on the length of preoperative HRT and RLE for SRS, but each individual situation is different.
|
||||
|
||||
Gender dysphoria has different orientations.
|
||||
|
||||
A person may value a woman's social identity and the approval of others, may value femininity in dress and outward physical characteristics, or may focus on first sexual characteristics.
|
||||
Gender dysphoria directed towards all of these aspects is often present in varying degrees in a person, but may also be relatively mild or non-existent in some areas.
|
||||
|
||||
At the same time, the level of need varies between individuals.
|
||||
|
||||
Some individuals are unable to accept living in a non-SRS body, some individuals are able to live normally for a period of time despite strongly seeking SRS, while for others SRS may simply be an option that makes life more comfortable.
|
||||
There are individuals who have lived with a stable female appearance and identity for a long time, but who may sometimes have trouble with their biological or legal gender, and who may (or may not) have the will and opportunity to choose SRS in due course based on their own arrangements.
|
||||
There are also individuals whose gender identity has been repressed for a long time and who are tormented by their aversive first sex, when immediate SRS may be the better option for them even if they have not experienced RLE or HRT, and they are capable of making a reasonable judgement based on their situation.
|
||||
However, there are also individuals who have unrealistic illusions about the effects of SRS and are disappointed or even regretful that they have not experienced the desired change in social identity after the procedure.
|
||||
|
||||
It is important for everyone to recognise their own psychological aspirations and to make rational choices about the pros and cons. After all, SRS is often a straightforward solution to the problem of first sex, but it does not directly address other aspects of gender dysphoria, and just like HRT, the decision to undergo SRS is irreversible, as is the time spent in distress and suffering.
|
||||
|
||||
The harsh reality is that, even after a complete transformation, we are only nearly given what the other half of the planet was born with. Beyond the completion of the SRS, we have other aspects of transformation and, more importantly, all other goals and pursuits in life. It is probably unwise to see the SRS or transformation as the only goal in life, and to sacrifice other important opportunities for it.

## Introduction to surgery
|
||||
|
||||
Chet's technique is similar to [Suporn]({{< ref "suporn" >}}), using the scrotal skin flap as the inner vaginal wall and labia majora, the foreskin and urethral mucosa as the vaginal vestibule and inner labia minora, the penile skin flap as the outer labia minora and the glans as the clitoris. In addition, part of the glans is also retained in the vaginal vestibule as a secondary sensitive organ, while Chet also attaches the corresponding nerves to the clitoral foreskin and tether, the labia minora and tether, and near the prostate. For those with insufficient material, Chet can also use the inguinal skin as a vaginal lining (at an additional cost). Unlike Suporn, Chet also accepts SRS with the colon and 0-depth SRS, and usually uses a single surgical shaping, which also results in a posterior labial union that cannot be resolved in a single operation.

||||
Based on the same technique, Chet also has a higher risk of complications, sometimes granulation and vaginal stricture, and has a similar requirement for three 50-minute dilation sessions per day. Likewise, Chet also has the need for perineal hair removal. Overall, Chet's technique is similar in appearance to Suporn and is a good choice as it is more conservative than Suporn in terms of vaginoplasty etc. and less likely to cause serious problems.

|
||||
For patients who do not have sufficient scrotal skin due to previous orchiectomy, hormone replacement therapy and many other factors resulting in skin atrophy, additional skin grafts can be obtained from the groin and joined to the scrotal skin to complete the required skin graft. Thus, even patients with SRS who have a small and short penis size can expect to have a good depth of pubic area.
|
||||
|
||||
The urethral opening is set in the correct anatomical position above the vaginal opening to allow for the female urinary flow pattern. All the cavernous muscles surrounding the urethra are completely removed to prevent tissue engorgement during sexual arousal, which may narrow the vaginal opening or cause difficulty during intercourse. By retaining the specific secretory glands around the urethra and vaginal opening, individualised self-lubrication can also be expected.
|
||||
|
||||
To ensure that the grafted skin is 100% hair free, the prepared skin pieces are carefully examined by light and any hair follicles that may remain are meticulously extracted before the grafting tube is inserted into the newly constructed vagina.
|
||||
|
||||
## Appointment matters

## Introduction to surgery

Kamol offers 0 depth, PI flap, colonic and peritoneal SRS.
|
||||
|
||||
After general anesthesia, a neovaginal dissection is performed along Denonvilliers fascia between the rectum and the urinary tract to a depth of approximately 5 to 7 inches. The flap SRS uses the skin of the penis and scrotum as the vaginal lining and the vaginal opening, or, if insufficient material is available, a slice of inguinal skin. Peritoneal SRS uses laparoscopic removal of the peritoneum as the vaginal lining and penile skin as the vaginal opening. Relatively adequate secretion can be achieved, while requiring less dilation. The core and fascia of the penis will be removed and the sensory nerves will be preserved for the creation of the clitoris and clitoral hood. With the testicles removed, the urethra will be shortened and deformed and the spongy tissue will be removed to allow urine to flow out like a woman.
|
||||
|
||||
For colonic and peritoneal SRS where laparoscopy is used, significant scarring at the surgical site can be avoided, but 4 wounds the size of a small thumbnail will be created on the stomach. As this technique is relatively new, there is a greater risk of complications.
|
||||
|
||||
Visually, the shape of the labia and clitoris in the Kamol technique is relatively unnatural, especially as the vaginal opening is often completely below the labia minora, the vaginal vestibule is contiguous with the surrounding skin, or even the labia minora meet above the vaginal opening.

A unique non-PI approach is used. Specifically, all the tissue of the glans is preserved, while the foreskin is used as the clitoral foreskin and the inner labia minora to indicate optimal sensitivity: part of the glans is used to shape the clitoris, the other part is buried at the foot of the clitoris as a secondary sensitive organ, while part of the foreskin is used to shape the clitoral foreskin and tether; the lower part of the vaginal opening is made with a flap at the perineum, the upper part with a flap of the urethral skin that is turned out, and the lateral The remaining scrotal/penile flap and urethral mucosa are sutured into a tube as the vaginal wall, with the subcutaneous tissue, including the hair follicle, stripped to ensure that there is no hair; the lateral scrotal flap is taken as the labia minora, and its junction with the lateral labia minora flap is sutured to the deeper tissue to ensure that the two are separated. It is difficult to achieve perfect results in a single operation, as the natural sublabial union and labial ties cannot withstand the frequency of expansion required in the early post-operative period, so a single SRS cannot create the appearance of the labia minora naturally wrapping around the underside of the vaginal opening, but only wraps the sides of the vaginal opening in a 'Λ' shape. ## ¿Por qué hacer la CRS? ¿Debería plantearme la CRS?

La disforia de género dirigida a todos estos aspectos suele estar presente en diversos grados en una persona, pero también puede ser relativamente leve o inexistente en algunas áreas.
|
||||
|
||||
Al mismo tiempo, el nivel de necesidad varía entre los individuos.
|
||||
|
||||
Algunos individuos son incapaces de aceptar vivir en un cuerpo sin CRS, otros son capaces de vivir con normalidad durante un periodo de tiempo a pesar de buscar fuertemente el CRS, mientras que para otros el CRS puede ser simplemente una opción que hace la vida más cómoda.

Hay individuos que han vivido con una apariencia e identidad femenina estable durante mucho tiempo, pero que a veces pueden tener problemas con su género biológico o legal, y que pueden (o no) tener la voluntad y la oportunidad de elegir la CRS en su momento en función de sus propias disposiciones.
