1. 以心理认同的性别至少生活一年以上（RLE）
|
||
1. 易性症诊断：易性症证明（任何三甲开具的诊断证明都可以），并证明无其他心理疾病
|
||
1. 公证证明：公证处开具，证明本人及至少一位直系亲属同意行性别重置手术
|
||

1. 无需 犯罪记录证明

1. 无需 未婚证明
